They collaborate with school staff to identify and assist at-risk students through individualized care. Comprehensive behavioral health assessments using screening toolsDevelopment of individualized treatment plans with student and caregiver inputProvision of individual, group, and family therapyBehavioral health interventions to support emotional, cognitive, and social functioningCrisis intervention and managementTreatment plan reviews to monitor progress toward student goalsFamily & Community Engagement Social Navigators strengthen connections between families, schools, and community resources.
