Qualifications:
- Valid Michigan LARA mental health professional license in the following areas:
- Licensed Masters Social Worker (LMSW) with capacity to provide Licensure & Medicaid Billing Supervision
- Experience implementing research-based strategies to address the social, emotional, and mental health needs of students
- Experience providing direct therapy/counseling services to students
Key Responsibilities:
Student Support
Social Navigators support students' attendance, behavior, and social-emotional development through targeted behavioral health interventions. They collaborate with school staff to identify and assist at-risk students through individualized care.
- Comprehensive behavioral health assessments using screening tools
- Development of individualized treatment plans with student and caregiver input
- Provision of individual, group, and family therapy
- Behavioral health interventions to support emotional, cognitive, and social functioning
- Crisis intervention and management
- Treatment plan reviews to monitor progress toward student goals
Family & Community Engagement
Social Navigators strengthen connections between families, schools, and community resources. They help caregivers navigate school systems and build positive relationships to support student success.
- Family therapy (with student present)
- Psychoeducation for parents/caregivers on diagnoses, behavioral health needs, and available supports
- Case management and care coordination
- Referrals to Community Mental Health and other external services
School Climate and Culture
Social Navigators promote restorative practices, SEL skill-building, and mindfulness strategies to foster a safe and supportive school climate. They contribute to school-wide efforts that enhance student engagement and well-being.
- Behavioral health interventions that support SEL and school engagement
- Psychoeducation and skill-building for students
- Crisis response and de-escalation to maintain a safe environment
- Support for school-wide mental health and wellness initiatives
Professional Collaboration
Social Navigators work as part of a multidisciplinary school-based team and maintain accurate and confidential records while collaborating with leadership to ensure effective service delivery.
- Participation in team meetings, case reviews, and professional development
- Documentation of assessments, treatment plans, services, and referrals in compliance with HIPAA/FERPA
- Submission of medicaid billing for eligible students
